Screenings and Exams
Physical Exams
Physical examinations are required by State Law in Grades K/1-6-11.
You may have your child examined by the school doctor or by your family physician.
It is often easier to make appointments when the students are home during summer vacation, therefore, for your convenience, forms are sent home near the end of school if your child is entering into grades K/1-6-11 for the upcoming school year.
If you choose to have your child examined by your family doctor, the completed private physical report should be returned to the school as soon as possible.

Dental Exams
Dental exams are required by state law for Grades K/1-3-7.
You may have your child examined by the school dentist or by your family dentist.
It is often easier to make appointments when the students are home during summer vacation, therefore, for your convenience, forms are sent home near the end of school if your child is entering into grades K/1-3-7 for the upcoming school year.
If you choose to have your child examined by your family dentist, the completed private dental report should be returned to the school as soon as possible.

Height and Weight
Height and Weights are required by state law for Grades K-12.
Your child's BMI (Body Mass Index) is based on the ratio of height to weight. BMI is reported as a percentile ranking based on the child's age and gender. The CDC created the percentile ranges to identify children who may be at risk for special health problems.
Vision Screenings
Vision screenings are required by state law for Grades K-12.
Vision Screening is done by the school nurse with the Titmus Vision Screener.
Parents/guardians will be notified if their child fails the exam.
Hearing Screenings
Hearing Screenings are required by state law for Grades K-1-2-3-7-11
Parents/guardians will be notified if their child fails the exam.
